We are happy to register new NHS patients within our catchment area.
How to Register
To register with our practice you will be required to complete two registration forms per person: one is the standard NHS GP registration form (GMS1) and the second is a new patient practice registration form. Please use the appropriate forms from the links below:
After you have completed the forms you must attend the surgery within 7 days with the completed forms and two forms of identification. Alternatively you can email the completed adult or child questionnaire form/s as appropriate to firstname.lastname@example.org
Please ensure that you have checked your postcode is within the practice catchment area prior to completing any forms.
Due to the current Coronavirus outbreak, we are currently accepting ID and proof of address via email however you will be asked to bring in your ID to your first appointment at the surgery.
Please complete and sign ALL relevant parts on the registration form. Once completed this can be sent along with ID and proof of address (if possible) by scanning or taking a clear picture of the documents and return to email@example.com
Acceptable Identification; passport, driving license, birth certificate, tenancy agreement, mortgage agreement, bank statement, recent utility bill, etc. All documents must be dated within the last 3 months. We are unable to accept mobile phone bills as proof of residence.
Until you have submitted your ID you will not be registered at the practice.
You must complete the health questionnaire as fully as possible before you come for your first appointment. You will be expected to make an appointment to attend the practice nurse for a new registration health check. This lest us know the important details of any past medical problems and means we can work together to get the best possible care. Please note that due to the current COVID-19 situation, we have suspenced new patient health check appointments until further notice.
